Next Steps

The next phase of the Hudson Valley Initiative will build on the success of the initial medical home practices to implement care coordination.  In 2010, the practices will be required to train staff and be certified in the Johns Hopkins University Guided Care program, conduct most of the elements of the Consumer Assessment of Healthcare Providers and Systems survey, and participate in expanded quality monitoring.


Our goal is to convey our story to others as a blueprint for  regions, states and multi-stakeholder entities engaged in pursuing new, better coordinated models for health care. The key to success is a shared vision among all stakeholders—providers, health plans and community leadership--for revolutionary health care transformation to improve the quality, safety and efficiency of health care. We are unique in our physician leadership of the  transformation process and in our commitment to research and evaluation, and also in the active participation in governance across the health care continuum, from small to large practices and federally qualified community health centers.
